Chronic Special Needs Plans (C-SNP)

If you’re dealing with a certain condition like diabetes, lung disorders, end-stage renal disease, or heart disease, it’s crucial to have access to the right care and doctors. We can help you join a Medicare Advantage plan that will cover extra care and everyday needs to help you stay as healthy as possible. While living with a special condition is hard, a customized health insurance plan and resources can make things easier!

For instance, C-SNPs with diabetes benefits may include supplies to monitor a patient’s blood glucose and insulin. The plans can also connect you to specialists to support your medical needs.

Before enrolling, you must have a confirmed diagnosis from a doctor of your qualifying condition and submit it to the official Medicare office.
