What C-SNPs CoverC-SNP: Common Qualifying Conditions
You must have a certain illness or a chronic condition to be considered for a C-SNP. Specific insured illnesses include:
- Autoimmune disorders
- Cardiovascular disorders
- Chronic heart failure
- Diabetes mellitus
- End-stage liver disease
- End-stage renal disease (ESRD)
- Severe hematologic disorders
- Chronic lung disorders, like asthma or bronchitis
- Chronic and disabling mental health conditions
- Neurologic disorders
Commonly-linked conditions may also be grouped together for even more specific benefits. These groups include:
- Group 1: Diabetes mellitus and chronic heart failure;
- Group 2: Chronic heart failure and cardiovascular disorders;
- Group 3: Diabetes mellitus and cardiovascular disorders;
- Group 4: Diabetes mellitus, chronic heart failure, and cardiovascular disorders; and
- Group 5: Stroke and cardiovascular disorders.

How Much C-SNPs Cost?
C-SNPs may charge a low monthly premium, and plan members will continue to pay the standard premium for Medicare Part B. Your plan may set its own deductibles, copays, and other cost-sharing for healthcare services (like home health, DME, and inpatient hospital care). However, SOME plans may have $0 monthly premiums, $0 deductibles, and $0 copays.
Enrolling in a C-SNP
If you meet the C-SNPs eligibility requirements, you may qualify for a Special Enrollment Period (SEP) — during which you can enroll in the plan that serves you. In other words, you can enroll at any time, as long as you have the condition.
Before enrolling, you must have a confirmed diagnosis from a doctor of your qualifying condition and submit it to the official Medicare office.
